For our Infusion Center patients, in order to save some time on your visit to the clinic, we ask that you complete our Patient Consent Forms and have them ready and with you when you arrive for your appointment. If you are receiving IVIG, Remicade, Rituxan, Vivitrol or Xolair therapy, there are specific consent forms that need to be completed. Please select your appropriate therapy. If you are receiving any other type of therapy, please complete the regular Patient Consent Forms. If you have any questions, please contact your local infusion center.
