Paragon Healthcare’s Home Cardiac Program offers a comprehensive and patient tailored approach, focused on the complex needs of the advanced CHF patient. Our patient population is unique, comprised of patients awaiting cardiac transplants, LVAD implantation and patients seeking relief of their advanced heart failure symptoms.
Our goal is to improve our patient’s quality of life. Our specialized expertise and dedicated Heart Failure, Heart Transplant and LVAD team of nurses are available to manage patient care and outcomes. Prior to discharge, clinical liaisons are available to provide highly customized “one on one” education on home INOTROPE infusion in the hospital to make the transition home as smooth and comfortable as possible.
Clinical indicators are monitored to reach desired goals and decrease the incidence of readmissions into the hospital and by decreasing the frequency of ER visits due to decompensated heart failure by providing ongoing patient education, close monitoring and management by our Cardiac Team Clinicians.
Home health nurses are available for biweekly visits to the patient’s home for cardiac assessment and ongoing patient teaching, as needed.
We have multiple partnerships with renowned physicians to achieve goals and improve outcomes. Our pharmacists provide therapeutic drug monitoring and work closely with physicians and cardiac team to facilitate planned treatment outcomes.

At Paragon Healthcare, we are committed to our patients and to making their treatment successful. With Paragon’s antibiotic therapy treatment, our patients can often discharge from the hospital sooner, and may even prevent against future hospitalization.
Paragon antibiotic services can be provided in the home setting and/or in one of our comfortable, convenient infusion centers where you feel at home.
In the home setting, every new referral is managed by a clinical nurse to ensure the highest quality patient care. Prior to discharge, clinical liaisons are available for one-on-one bedside teaching for patients and their care givers so they can easily learn to administer home IV antibiotics. Support and training from our clinical liaisons help make the transition home as smooth and comfortable as possible.
Once at home, our commitment to our patients continues. All after hour calls are answered by live clinical staff to ensure quick response to patient care. Our pharmacists work with physicians in obtaining and managing lab results and ensure that physicians receive a copy. Medication dose times can also be adjusted to accommodate the patient’s schedule, ensuring rest, healing and achievable lab draw times. We also monitor clinical indicators to reach desired goal and decrease the incidence of readmissions into the hospital.
Our preferred medication administration method is an elastomeric device, known as the “infusion ball”. Elastomeric devices are easy to learn and they help to decrease medication errors as well as the number of nursing visits needed.

Our pharmacists are trained to provide equal analgesic dosing to provide a smooth transition from oral or transdermal pain medications to IV or SubQ pain management. IV and SubQ pain management provide steady and continuous pain relief. We are able to provide highly concentrated bags at any size by using a base powder.
Hospice patients take immediate priority in the pharmacy. Their medications are prepared immediately and delivered to the bedside as soon as possible to provide pain relief. Our pharmacists provide therapeutic drug monitoring and work closely with the hospice team and family to ensure that the patient is as comfortable as possible and has adequate supplies.
